Important information and disclaimers

Total medication cost is the negotiated rate an in-network provider currently charges for the item or service. This rate includes any dispensing fee and tax. Total medication cost may include a deductible, a percentage of the prescription cost, a fixed amount or other charges. Plus, a balance, if any, paid by your benefit plan. Your estimated price is the amount you pay to get the prescription under your benefit plan.

Your search results include in-network pharmacies only. If you use an out-of-network pharmacy, you may be responsible for the full price of the item.

The prices provided in this tool are estimates only and not a guarantee of payment amount or benefits. Estimated usage, price and savings are calculated through the current benefit plan year or through the end of the current calendar year. Pricing may change based on the quantity and/or pharmacy provider, where applicable under your benefit plan design. Your plan coverage and estimated pricing are shown for the searched drug, applicable generic equivalents and potential alternatives. The generics and alternatives are provided for informational purposes only and not intended to substitute or replace clinical decision-making. For medical questions, please consult with your prescriber. CVS Caremark, including its affiliated entities, disclaims liability to the fullest extent of the law for any treatment decisions made as a result of this information. All benefit decisions, including any changes to coverage or cost-sharing, are made solely by your health and/or pharmacy benefit plan sponsor (“Plan Sponsor”). CVS Caremark does not determine the scope or design of benefit coverage. Preventive items and services, as defined by your plan benefit, won’t be subject to cost sharing when dispensed by an in-network pharmacy. Depending on your benefit plan, copayment assistance and other third-party payments may be counted in the calculation of the deductible and out-of-pocket maximum. Copayment assistance for certain specialty medications might not count towards the calculation of the deductible and out-of-pocket maximum. CVS Caremark does not administer certain plan designs in all states. If you have questions about your plan or participating pharmacies, please contact the number on your member ID card. Please contact the pharmacy directly prior to seeking services to confirm the pharmacy’s participation in the plan’s network and the member’s health benefit plan.

Please be advised that certain medications displayed as “Covered” on Caremark.com may not be covered under your prescription benefit plan. These medications may instead be available through the RxSavingsPlus for Non-Covered Drugs Program, which provides access to discounted pricing but does not constitute “coverage” under your plan benefit. Medications accessed through the RxSavingsPlus for Non-Covered Drugs Program do not apply toward plan accumulations, including deductibles, out-of-pocket maximums, or other cost-sharing thresholds and are not eligible for reimbursement under the plan benefit. RxSavingsPlus for Non-Covered Drugs Program is not available in all states. For coverage details, please contact the number on your member ID card.

A National Drug Code (NDC) is a unique 10- or 11-digit number that identifies every drug sold in the U.S., including its name, strength, and dosage. You can usually find it on your prescription label or medication package, near the drug name or barcode.

When using this tool to search by NDC, enter only the numbers, not the dashes.
